If you would like to remain anonymous, you are not required to submit your name or pets name.
Any client that submits a survey with their information will receive a $5.00 credit on their account.
(maximum $5.00 every 6 months)
(however we would love to have you fill one out after each visit)
There is a separate survey for our boarding clients, grooming clients and those that came by for a tour of our facility.
On all surveys, please rate all of the questions with 1 being the lowest and 10 being the highest. All comments are greatly appreciated.
Only the manager and assistant manager will have direct access to the results of these survey's.
